Athletic Physicals to Start May 7

Dear Argyle Athletic Parent,
The Argyle athletic programs require every athlete to complete an annual physical. The Argyle I.S.D. athletic physicals will be conducted Saturday May 7, 2016 at Orthopedic Associates. The physicals will start at 9:00am for all upcoming 9th-10th (Current 8-9th) grade athletes and 10:00am for all upcoming 11th- 12th (Current 10-11th) grade athletes of Argyle ISD.
Please acknowledge the following concerning the physicals:
- When: Saturday May 7, 2016. Grades 9-10 will be from 9-10am and grades11-12 will be from 10-11am. (student grade is based on the upcoming 2016-2017 school year)
- Where: Orthopedic Associates located at 5000 Long Prairie Rd Flower Mound, Tx 75028
- Cost: $20.00 donation per physical with either cash or check (checks made payable to Argyle ISD).
- Who: All male and female 2016-2017 9-12 athletes.
- Be sure to completely fill out the Medical History portion of the UIL Pre-Participation form with all student/parent signatures.
- Bring the completed Medical History portion of the UIL pre-participation form when purchasing a ticket.
- Tickets can be purchased the week of April 25th-29th and May 2nd-4th from 8am-11am and 4pm-5pm in the Athletic Training office located in the Argyle ISD boys Fieldhouse.
- When you show up for your physical, you will be checked in. If it is not filled out, then a parent/guardian will have to be present to complete the paperwork before you will be allowed to get your physical. It is more convenient to complete the medical history portion of the physical before you show up on May 7th.
- Only UIL Athletic Physicals will be done on May7th. Any physical examination for another organization or event, will not be completed.

All student athletes for 2016-2017 school year must have a completed physical on file with the Athletic Trainer prior to your first day of workouts in August. All other UIL paperwork can be completed online at another date TBD. Argyle ISD is very fortunate to have several healthcare professionals and physicians to donate their time to assist us with our athletic physicals.
